Description As a part of an interdisciplinary team, the Social Worker coordinates social services to assist patients and their families in maintaining or improving their physical, mental, and psychosocial needs. Provides direct social work assessments, problem identification, goal setting, and implementation of care plans. He/she collaborates with all disciplines to optimize patient outcomes; scope of services may include but is not limited to developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.
Carries out responsibilities in accordance with Baltimore Medical System Core Values, policies/procedures, and applicable civil laws. Is accountable for efficiently using resources and supporting the BMS mission.
Requirements Education: - Bachelor's degree in Social Work
- Master's degree in Social Work preferred
Experience: - Minimum of two years (of which two years is health care related field).
- Must have expertise in one or more of the following areas:
- Clinical: Advanced social work knowledge of psychodynamics with a broad application of SW theory.
- Program Coordinator: Knowledge of patient population needs and understanding of program elements required to mitigate deficits.
